At Chicago Smile Design, we feel that every patient deserves to have a beautiful, healthy confident smile!  We are excited to offer our patients a unique program we call Whitening for Life. When you come to our office for your preventative examination, x-rays and professional cleaning, we will provide you with customized bleaching trays and materials for a one-time enrollment fee.

Then, at each recommended preventative visit, we will provide you with a complimentary touch up kit of bleaching gel.  This ensures that you will be able to keep your teeth bright, beautiful and healthy for life! Our team loves to see you smile and takes pride in helping patients maintain the highest level of oral healthy possible.

Responsibilities of Chicago Smile Design

Our team at Chicago Smile Design will create custom whitening trays for our patients who are currently 18 years old or older and are able to dentally and medically receive the tooth whitening services.

1. The patient will be scheduled to have the custom impressions of their teeth made using the iTero scanner. This scan is used to create your customized whitening trays. They will be ready to be picked up in approximately 14 days.  
2. Once the trays are ready, our staff will provide the patient with the custom trays, as well as syringes of professional grade whitening gel. During this appointment, each patient will also be given full instructions regarding how to use the trays. Our staff will also answer any questions patients have about the trays at this time.

Responsibilities of the Patient

The following responsibilities are what we expect from all patients who wish to take part in our Whitening for Life program.

1. The individual must be an active, current patient or become a patient of record. They should schedule their professional cleaning, as well as an exam before the impression appointment is made for optimum oral health and best results in whitening.
2. It is critical for patients to always keep their regularly scheduled x-ray, exam, and professional cleaning appointments to ensure their healthy smile.
3. If you have a change to your schedule, you will need to provide us with a 48 hours’ notice.  Also, we have made it a requirement to schedule your appointment within four weeks of your previous appointment date if you wish to remain qualified in the Whitening for Life program.
4. You will be given one whitening gel syringe at each of your professional cleaning appointments.  We recommend that the whitening gel is used closest to your cleaning appointment for best results. You can purchase extra syringes for a minimal fee.
5. If your tray becomes lost or damaged in any way, there is a reduced fee for replacements.
6. If you fail to meet even one of the rules outlined here, you will become disqualified for the Whitening for Life program. However, you will have the opportunity to re-enroll in the program at a reduced reactivation fee.
