About You

Make sure you are keeping your regularly scheduled x-ray, exam, and prophy appointments. If you want to take part in our Whitening for Life service, you have to be an active and current patient. Make sure you are scheduling your appointment within four weeks of your previous one to stay qualified. Damaging or losing your tray results in a $35 fine, so be responsible!

The Steps

The Chicago Smile Design staff makes custom whitening trays for you. The trays are given after a regular exam and prophy. A type convenient for you will be scheduled to make custom impressions of your teeth, which is then used to create the whitening trays. Roughly two weeks after the impressions are made, they will be available for pick-up. When they trays are ready for pick-up, you will receive the trays plus three syringes of professional grade whitening gel. Our staff walks you through the use of the trays.
